By completing the form, you are requesting that we share your Free and Reduced Price School Meal Application with any/all of the area community assistance agencies that you designate below to determine eligibility for other programs in which your child[ren] may qualify.

Should you wish to submit your request to share your application, you may download, print and complete this form by selecting the small, black icon in the upper right-hand corner of the document viewer below. Once you have completed your request, please return the form to the building that your child attends. You may also complete your form online.
