Community Assistance Enrollment

You may  complete an Assistance form online. By completing the form, you are requesting that we share your Free and Reduced Price School Meal Application with any/all of the area community assistance agencies that you designate below to determine eligibility for other programs in which your children may qualify. Once you have completed your request, please return the form to the building that your child attends.
